To demonstrate the effectiveness of a family-centered, integrated health and social service network approach to reducing HIV/AIDS incidence and improving health outcomes among high-risk minority populations in transition from domestic violence, incarceration and substance abuse treatment, while addressing and combating the health and social barriers that may contribute to HIV/AIDS incidence among high-risk racial and ethnic minorities
